A PLACE TO CALL HOME:
Yae’s House of Evolution is a Transitional Independent Living Roommate-Style Home that will provide therapeutic treatment (via EMT), case management, and other psychosocial stabilization and growth opportunities to female youth and youth who have phased-out of the foster care system. Youth can be between the ages of 18-25 years old. Length of stay is between 90-180 Days, with the first 3 days usually being crisis management. It is our hope and mission to help young women find support, experience trust, and evolve.

PREVENTION:
Yae’s House of Evolution with Evolving Minds Therapy (EMT) will join the fight in preventing homelessness, addiction, prostitution, and trafficking among the cities most vulnerable youth.
SUPPORT:
Young women will learn to transition into society with confidence and skills for self-sufficiency and care. Groups, individualized therapeutic treatment and guidance, and other psychosocial supports will be provided to assist these youth in becoming the best version of themselves.
QUALIFICATIONS:
*Women
*Male To Female Transitioning (Completed Transition) Accepted
*Ages 18-25 years old. Ages 26-27 will be screened to see if the home and services fit the individual’s needs.
*Must be Medication Compliant
*Must be drug-free
*Must have income or be willing to work, or attend vocational placement program
